APPLICATION FORM TO DATA CONTROLLER
General Explanations
Personal data owners (“Applicant”), who are defined as the relevant person in the Law No. 6698 on the Protection of Personal Data (“PDPL”), are granted the right to make certain requests regarding the processing of their personal data in accordance with Article 11 of the PDPL.
The application form has been prepared to determine your relationship with OZBİLİM TEXTILE MACHINERY IND. TRADE CO. LTD. (“OZBİLİM MACHINERY”), to determine your personal data processed by OZBİLİM MACHINERY, if any, in full, and to respond to your application correctly and within the legal period. In order to ensure the security of your personal data and to prevent unlawful data transfer, OZBİLİM MACHINERY may request additional information for identification and authorization. If the information provided by the Applicant is not correct and/or up-to-date or the requests are unauthorized, the responsibility for this matter belongs to the Applicant.
If the Applicant's application is to be answered in writing in accordance with Article 7 of the Communiqué on the Procedures and Principles of Application to the Data Controller, no fee will be charged for up to ten pages. A processing fee of 1 Turkish Lira may be charged for each page over ten pages. If the answer to the application is given on a recording medium such as a CD or flash memory, a fee equal to the cost of the recording medium may be charged.
Scope of the Right to Apply Within the Framework of Article 11 of the KVKK
The Applicant may apply to ÖZBİLİM MAKİNALARI and make requests on the following issues:
1) To learn whether or not their personal data has been processed,
2) To request information regarding their personal data if they have been processed,
3) To learn the purpose of processing their personal data and whether they have been used in accordance with their purpose,
4) To learn the third parties to whom their personal data has been transferred domestically or abroad,
5) To request correction of their personal data if they have been processed incompletely or incorrectly and to request notification of the transaction made within this scope to the third parties to whom their personal data has been transferred,
6) To request deletion, destruction or anonymization of their personal data if the reasons requiring processing have been eliminated, despite the fact that they have been processed in accordance with the provisions of the KVKK and other relevant laws, and to request notification of the transaction made within this scope to the third parties to whom their personal data has been transferred,
7) To object to the emergence of a result against them by analyzing their processed data exclusively through automated systems,
8) Requesting compensation for damages in case of damages due to unlawful processing of personal data.
Application Method
According to the first paragraph of Article 13 of the KVKK; applications to be made regarding these rights must be submitted to us in writing and signed or by other methods determined by the Personal Data Protection Board ("Board").
Applications to be made in writing within this framework can be submitted to us by printing out this form;
With the personal application of the Applicant,
via a notary,
with a registered electronic mail (KEP) address, secure electronic signature, mobile signature or
by e-mail sent by the Applicant from the electronic mail address previously notified to us and registered in our system.
